As a new patient, we will need you to complete a few forms to get started. You can download and print our new patient forms here and complete them before you arrive. This is a big time saver and helps us get you seated in a timely manner. We will also need contact information for your primary care physician and a current list of any medications you are taking.

At White Wolf Dental we pride ourselves on providing the highest quality, most thorough dental experience anywhere. We use state-of-the-art dental imaging to create a detailed road map to your dental needs. Depending on what x-rays you bring or have sent from your previous dentist, we may take a set of intra-oral x-rays and a 3D x-ray scan.
Next, we perform a thorough oral exam including examination of soft tissues to determine gum health, screen for oral cancers, and diagnose any other issues. We take a set of photographs at this point to visually document the exam and our findings.
A cleaning is generally not performed at the first appointment as it is important to assess your needs and determine the most appropriate course of treatment before proceeding.
After your exam, we will schedule a follow-up appointment to work with you to make a plan for how to proceed with your treatment. This follow-up consultation is always free-of-charge and the goal is to come to a mutual understanding and create a plan of care that fits your needs and budget. It is very important to us that you understand your dental needs and work with us as a partner to achieve excellent dental health.
